Programs and Specialized Services
Secure Alzheimer’s dementia care unit
Heritage Grove has a 23-bed unit for residents with dementia who need a secure environment that allows for freedom of mobility and access within it. There are indoor and outdoor areas that provide a variety of options for stimulating, safe activity. Certified nursing and activities assistants provide one-to-one assistance for all activities of daily living. Licensed nurses provide skilled assessment, treatment and intervention for medical care needs. Social workers work closely with the nursing team to ensure that psychosocial needs are met and family involvement is maintained. Physicians and psychiatrists with specialization in geriatric care make weekly rounds to provide assessment and supervision of care and treatment.
Specialized rehabilitation services—certified Medicare unit
Individuals with delirium, dementia or mental illness often need a specialized environment that can address challenging behaviors during recovery from surgery or serious illness. Heritage Grove provides a seven-bed short-term stay unit for individuals with behavioral health care needs who require skilled nursing care and rehabilitation following a hospitalization. Licensed nurses, nursing assistants, physical, occupational and speech therapists work with social workers and mental health professionals to assure well-managed recovery and appropriate return to community placements.
Mental health evaluation and stabilization program
Heritage Grove maintains a private evaluation room so that individuals can be admitted for thorough assessment, stabilization and follow-up placement. Individuals are admitted from throughout central and southeastern Washington to receive medical, psychiatric and functional assessment by an interdisciplinary team. Under the direct supervision of a geriatric psychiatrist, nurses and social workers complete a battery of medical tests, screenings and observational evaluations to assist in diagnosis and interventions. A comprehensive behavior management plan is developed, which becomes a part of the discharge packet. After stabilization, the individual returns to the appropriate level of care to maintain functioning.
Specialized long-term care—
Integrated psychiatric and medical services
Heritage Grove offers long-term care for individuals with chronic medical and/or behavioral health conditions that require 24-hour skilled nursing care and psychiatric treatment. Individuals with Parkinson’s, Huntington’s, ALS, schizophrenia, bi-polar disorder, vascular dementia and other conditions that effect brain function frequently have very complicated health issues that require both medical and psychiatric assessment and treatment. Care is coordinated among psychiatrists, neurologists, primary care physicians, nursing staff, social workers and rehabilitation therapists to assure a comprehensive treatment plan. A wide variety of therapeutic activities and both individual and group counseling is provided within the facility to maintain independent functioning and quality of life. Family involvement is encouraged and supported in all aspects of resident care.
