Learn more about the My HEALTH risk assessment by reviewing the frequently asked questions below.
What is a health risk assessment?
A health risk assessment is a brief quiz that will help you learn more about your health risks. Once you complete the questionnaire, you'll immediately receive a customized health report that suggests changes that you may want to consider making to improve your health.
Why should I take a health risk assessment?
By taking a health risk assessment, you'll learn more about your health and risk factors that have an effect on it. You may also learn about areas of your health that can be improved to help you avoid more serious problems and live a longer, more productive life.
What kind of questions will you ask me?
The health risk assessment asks general questions about your current health, such as cholesterol level, blood pressure, height and weight. It also asks questions about lifestyle choices that may affect your health, such as your exercise habits and your use of tobacco and alcohol.
What if I don't know my cholesterol level or blood pressure?
Don't worry! You don't need to fill in all the information to complete the health risk assessment. Of course, the more information you can provide the more accurate report of your current health age you'll receive.
Will my health information remain private?
Yes! Your health information will remain completely private.
What should I do if my health isn't as good as I want it to be?
If you'd like to be healthier (and who wouldn't?), your personal health risk report will give you some tips that can help. But, it's also important for you to see a doctor before making major changes that could affect your health, like beginning an exercise program. If you don't have a doctor, we would be happy to help you find one. Just call 509-249-5099 or visit: Find a Doctor.
